Wexford 4-H Forms

 Apply to Volunteer with 4-H!

Looking to get involved?  Fill out this application and send it to our office!  This is our screening process to keep our kids safe.  If accepted as a volunteer, we will then have a conversation about how you can be involved: starting a club, helping an established club, leading specific projects, organizing short-term activities, helping out at our Proud Equestrian Program, chaperoning, mentoring, helping at state or regional events, or something else that may fit our needs and your talents.

Fundraising Application and Report

Any 4-H entity that wants to hold a fundraising event needs to fill this out first!  Drop it off at the 4-H office for approval.

Annual Club Financial Report

Every club needs to fill out a financial report every year.
